I wish my name to appear on the Stand Fascia as follows:

*** Unless this form is returned before dead line date of exhibition, the name appearing on the contract will be used on the same panel and any changes to the company name on the fascia will incur a (€ 40) additional charge per name. Any changes to the company name on the fascia after the form has been submitted will incur a (€ 40) additional charge per name.

PLEASE BE NOTED THAT NO OTHER NAME (S) & LOGO (S) THAT ARE DIFFERENT FROM THE ABOVE-MENTIONED ONES CAN APPEAR ON THE FASCIA BOARD.
